Position Summary

Ready2Nurse is seeking an experienced Registered Nurse (RN) Field Case Manager to join our high-performing team in Santa Clara and Alameda Counties.

This full-time role is ideal for a skilled, motivated RN with 3+ years of experience who wants to deepen their home health case management expertise and grow into a clinical leadership position.

You’ll partner directly with the founder and Director of Patient Care Services to coordinate patient care, lead field clinicians, and ensure excellence across every episode of care.

Key Qualifications:

  • Experienced Registered Nurse (RN) with proven ability to deliver high-quality, compassionate patient care
  • Case management experience preferred, including coordination of care plans and interdisciplinary communication
  • Clinical field experience in home health or community settings strongly valued

Key Responsibilities

  • Perform comprehensive patient assessments and develop individualized plans of care.
  • Coordinate and oversee all aspects of the patient’s episode, ensuring continuity between clinicians, physicians, and caregivers.
  • Conduct in-home patient visits as the primary clinician, providing skilled nursing assessments, direct care, and coordination of services throughout each episode of care
  • Review OASIS documentation for accuracy, compliance, and timely submission.
  • Mentor and support field nurses and therapists, promoting best practices and clinical consistency.
  • Identify and address early signs of decline to prevent hospitalization and improve outcomes.
  • Serve as the primary clinical liaison between the care team and agency leadership.
  • Document all visits and communications in WellSky (Kinnser) EHR accurately and efficiently.

Qualifications

  • Active California RN license and current CPR certification (required).
  • Minimum 3 years of professional nursing experience (non-negotiable).
  • Case management experience in home health or related setting (preferred).
  • Field experience delivering direct in-home patient care (preferred).
  • Proven ability to lead, organize, and communicate effectively with multidisciplinary teams.
  • Strong clinical judgment and attention to documentation accuracy.
  • Valid driver’s license, auto insurance, and reliable transportation required.
  • Smartphone with GPS capability required for visit documentation.
  • Fluency in Mandarin or Vietnamese a strong plus.

Compensation & Schedule

  • Full-Time, Salaried Position
  • Base Pay: $140,000 – $160,000 per year (commensurate with experience and leadership capability)
  • Bonus/Incentive Structure: Additional performance-based bonuses available; total compensation may exceed base range for top performers
  • Comprehensive benefits package including health, mileage reimbursement, and paid time off
  • Geographic Focus: Primarily Santa Clara County, with occasional coverage in Alameda County
  • Schedule: Monday–Friday, full-time availability required
