Job Title: Mobile Wound Care NP – AGNP / CNP | Certification Paid, No Clinic Walls

Company: Twin Cities Physicians

Compensation: $105,000 to $120,000 annually

Benefits: Full Benefits Package (for full-time status)

Location: Greater Twin Cities Metro Area, MN

Schedule: Full-Time

About Twin Cities Physicians:
We are a Minnesota-based rounding physician group delivering primary care directly to residents in senior communities. Our model keeps people out of the ER and empowers providers with flexibility and autonomy.

Position Summary:
We’re hiring a Nurse Practitioner (AGNP or CNP) to join our mobile care team with a focus on wound care. Whether you’re certified in wound care or want to be—we’ll pay for your training and certification. Enjoy the freedom of working independently without the pressure of a traditional clinic.

Key Responsibilities:

  • Travel to and treat patients in assisted living and memory care settings
  • Evaluate wounds, manage chronic health conditions, and coordinate advanced care
  • Collaborate closely with community staff, physicians, and families
  • Document thoroughly and communicate proactively

Qualifications:

  • CNP license in the state of Minnesota
  • AGNP certification strongly preferred
  • Experience in geriatrics, wound care, or internal medicine
  • Must be comfortable working independently and traveling to patient sites
